Our Story

Unity Health Navigators was built on a core belief: solutions are most effective when they are shaped by the people and communities closest to the challenges.

Across South Carolina, many individuals and families face barriers to achieving their best health — including limited access to care, transportation challenges, health disparities, stigma, and systems that have not always been designed with their needs in mind.

UHN was created to help bridge those gaps. We meet people where they are, lead with trust, honor lived experiences, and connect individuals and communities to the services, resources, and support they need to thrive.

By combining preventive health services, community partnerships, healthcare navigation, and culturally responsive outreach, UHN is working to humanize care, advance health equity, and increase access to care throughout rural and underserved communities.

    Nonprofit Status

Unity Health Navigators is a registered business through the South Carolina Secretary of State and is currently in the process of expanding our reach by obtaining 501(c)(3) nonprofit status to further strengthen our ability to serve communities, expand programs, and build sustainable partnerships. This additional service arm will allow UHN to deepen our impact, access critical rural health funding, and better advance our mission of bridging gaps in care and promoting health equity.
