                    Let's Build Together!
Partnerships are how we close the gaps that systems leave open.

Why Partner With
Unity Health Navigators

At Unity Health Navigators, we believe that meaningful change happens when communities, healthcare systems, public health agencies, and local organizations work together. We do not build transactional partnerships—we build transformational ones grounded in trust, equity, and a shared commitment to improving health outcomes for the communities we serve.

People fall through the gaps in care — not because solutions don't exist, but because systems are not connected— Jawana L. Williams, Founder

Experience You Can Trust

With more than 12 years of experience as a public health practitioner, Jawana has designed, implemented, and led infectious disease prevention and care programs across public health, clinical, community-based, academic, and correctional health settings. Her expertise spans disease surveillance, HIV/STI prevention and care, program implementation, healthcare systems improvement, community engagement, health education, and care navigation. Throughout her career, she has developed innovative, equity-centered programs that improve health outcomes, strengthen systems of care, and expand access for underserved and disproportionately impacted populations.

System-Level Impact

Our work extends beyond direct service—we strengthen the systems that shape health. UHN's founder has successfully translated surveillance data into action by identifying gaps in care, targeting outreach for early detection, integrating HIV prevention and navigation services into clinical workflows, and building cross-sector partnerships that improve continuity of care. From correctional health systems to community-based settings, her work has consistently moved individuals from the margins into sustained care while advancing equitable, person-centered public health practice.

Transformational Partnerships

We do not believe in transactional partnerships—we believe in transformational ones. Health inequities persist not because solutions are unavailable, but because systems often operate in silos. UHN exists to bridge those gaps by connecting healthcare providers, public health agencies, community organizations, faith institutions, and local leaders to create coordinated, trust-centered pathways to care. Together, we move beyond referrals to build sustainable partnerships that improve health outcomes and strengthen communities.

Our Partnership Philosophy
At UHN, we believe communities thrive when organizations work together—not in silos, but in partnership. We value relationships rooted in trust, mutual respect, shared accountability, and a collective commitment to improving health outcomes.
