
We are more than a community-based organization—we are a mission-driven initiative committed to Bridging Gaps in Care and advancing healthier, more equitable futures. Our work integrates community outreach with data-informed strategies to engage populations disproportionately impacted by HIV and other infectious diseases and too often disconnected from prevention and care.
Our current initiatives include HIV and STI counseling, testing, referral, and education services; community-based biometric screenings; infectious disease consulting; and training and speaking engagements that support awareness, capacity-building, and health equity across diverse community settings. This work includes education on HIV prevention tools and intentional linkage to PrEP and other prevention and care resources through trusted community and clinical partners.
As we continue to grow, we are building the infrastructure for scalable, innovative programs—including mobile health services, carceral health partnerships, juvenile evidence-based sexual health initiatives, and expanded community collaborations. These efforts are designed to strengthen prevention pathways, support continuity of care, and advance the CDC’s Ending the HIV Epidemic goals throughout the Southeastern United States.
Nonprofit Status
Unity Health Navigators, LLC is a registered business and is currently in the process of obtaining 501(c)(3) nonprofit status to further strengthen our ability to serve communities, expand programs, and build sustainable partnerships. This designation will allow us to deepen our impact, access critical funding, and better advance our mission of bridging gaps in care and promoting health equity.
